Hypospadie, een sluitingsdefect van de penis, is een aangeboren afwijking die voorkomt bij één op de 200-300 pasgeboren jongens. De uitgang van de plasbuis ligt hierbij niet op de top van de eikel, maar in het traject tussen de eikel en de balzak. Kinderen met hypospadie worden rond hun eerste verjaardag geopereerd om de uitgang op de normale anatomische positie te krijgen. Indien nodig wordt dan ook een peniele kromstand gecorrigeerd. Het doel van de behandeling is dat jongens daarna met een naar voren gerichte straal kunnen plassen, een rechte penis hebben in erectie en een nagenoeg normaal uiterlijk van de penis. Echter, rondom de puberteit kan de gedane correctie voor nieuwe problemen zorgen met het plassen en wordt iemand seksueel actief. Het eindresultaat van de doelstellingen van hypospadie is derhalve pas goed op volwassen leeftijd te beoordelen. In dit proefschrift zijn jongens, geopereerd tussen 1987-1993, op 18 jarige leeftijd beoordeeld op gebied van hun plasfunctie, seksuele functie en het penisuiterlijk. Doorgaans hebben de hypospadie patiënten goede resultaten op de lange termijn, echter zijn er meer problemen bij jongens met een ernstigere variant van hypospadie vergeleken de mildere varianten van hypospadie op gebied van het complicaties, seksueel functioneren en het penisuiterlijk. Daarnaast is er reden om te denken dat het preoperatief stimuleren van de penis groei met testosteron geen schade brengt op de lange termijn, de voorhuid gespaard kan worden bij milde varianten en dat de ernstigere hypospadie patiënten ook met een enkele operatietechniek behandeld kunnen worden.
